Your First Visit
Please bring your insurance information with you and arrive about 10 minutes early so that you have enough time to complete any necessary forms. You can also download and bring the completed New Patient Form* to your appointment. Once you register with us, you will be able to use our Patient Portal.
*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
To start, your dentist will review your medical and dental history and complete a thorough dental examination of your teeth, gums, bite and jaw, any required x-rays, and an oral cancer screening.
Based on your examination and history, your dentist will create a long-term dental treatment plan for you. There will also be time dedicated to answering any questions you may have about your dental health and treatment options.

- How do I cancel or reschedule an appointment?
To cancel or reschedule an appointment, please call us. Please notify us of any change of plans as soon as you can, so another patient can use the space in our schedule.
- Do you offer appointment reminders?
In the days before your appointment, you will receive automated phone appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible to let us know.
- Do you provide sedation for dental procedures?
Yes, our office is able to offer oral sedation for patients who may experience fear or anxiety during dental procedures.
- Do you provide services in any languages other than English?
Yes, our office is able to provide dental services in several languages including French, Serbian and Croatian.
If you have a language preference, please advise us when booking your appointment.
- Do you offer direct billing to insurance?
Yes - for your convenience, we offer direct billing for most insurance plans. We are able to submit your insurance claim electronically, resulting in faster reimbursement, without the requirement for paper forms and signatures.
Our administrative team can help you understand your insurance plan better and can also send estimates to your insurance company prior to starting any major treatments.

- What are your payment methods?
We accept payments by cash, cheque, debit or most major credit cards. You are responsible for any co-pays or amounts not covered by insurance. These must be paid at the time of your appointment.
- Do you have an online Patient Portal?
Yes! Once you have registered with us, you can use your email to access our Patient Portal.
